A written account, after an incident is resolved, of what happened and what should change.
The useful ones are blameless by construction: they describe what the system allowed rather than who did it, because a review that stops at human error stops people telling you about mistakes. A postmortem that produces no change to a system, a process or a piece of documentation was a meeting.
