The meeting or document that examines an incident after service is restored, to decide what changes as a result.

Distinct from the technical root cause work: the review asks what the organisation should do differently, which frequently has no engineering answer at all. Where the cause was a decision nobody made - a certificate nobody owned, a forecast nobody read - the finding belongs here rather than in a ticket.
