The process by which planned changes are proposed, reviewed, scheduled and recorded.
It exists because most outages are self-inflicted and arrive on purpose, so a record of what changed is the fastest route to what broke. It fails in two opposite directions. Too heavy, and it delays small safe changes until they arrive in large risky batches - the process manufacturing the risk it was meant to control. Too light, and nobody can answer what changed at the hour the graph turned. The useful test is whether the record answers that question in one minute during an incident.
